What does the CO-Triage Assessment Provisional Referral Report include?

The CO-Triage Provisional Referral Report captures questions asked and patient answers, including interviewer comments recorded during the 10-minute in-person or telephone CO-Triage screening evaluation. The report organizes the patient’s clinical information according to the six dimensions of The ASAM Criteria. The report identifies a provisional ASAM Level of Care to which the patient should be referred for an ASAM CONTINUUM comprehensive assessment as well as any major medical and psychological conditions. The interviewer can then use this information to help identify where to send a patient to treatment.

The Final Report is an individualized, multi-page report that includes the following information:

  • A header – which includes the patient’s name, and the date and time the assessment began and ended.
  •  A provisional ASAM Level of Care recommendation – a recommendation of ASAM Level 0.5, 1, 2, 3 or 4 to which the patient should be sent to for an ASAM CONTINUUM comprehensive biopsychosocial assessment and more precise placement in a specific Level of Care (e.g. 3.1).
  •  Identification of sublevel care needs, such as co-occurring enhanced care, opioid treatment services, and more.
